Plastic Surgeon Advises Waiting for Breast Surgery after Weight Loss

Dr. Yale S. Popowich, a plastic surgeon in Portland, advises his breast surgery patients to consider waiting until they are near or at their ideal weight before having a breast enhancement procedure, which is substantiated by a recent study featured in the September issue of Plastic and Reconstructive Surgery. That study found that most patients who undergo breast reduction are initially satisfied with their results, but many of those who go on to lose weight after surgery become dissatisfied with the shape and contour of their breasts.

“Breast reduction surgery produces better long-term results if the patient is at a stable weight before surgery,” explains Dr. Popowich. “Weight loss and pregnancy are the most common physical changes that can affect the breast volume. Because breast reduction surgery can be just as much about reshaping the breasts as reducing the breast volume, any significant weight loss after the surgery may change the results. Significant weight loss after breast augmentation in Portland also can impact the results, since the shape and volume of the breasts are impacted by a woman’s weight.”

Breast volume can change if a patient loses even 10 or 15 pounds, but the greatest changes occur with patients who lose massive amounts of weight, either by diet and exercise or bariatric surgery, according to the study. Although most patients in the study reported being able to exercise more easily as a direct result of their breast reduction surgery, those who chose to undergo breast reduction before bariatric surgery were ultimately dissatisfied with how their breast appearance changed.

“Breast reduction allows many women to exercise more easily,” says Dr. Popowich. “However, if you lose a significant amount of weight after your procedure, your results may be altered. For that reason, if a woman tells me that she wants to undergo bariatric surgery or lose significant amounts of weight, I may recommend that she wait to have her breast surgery until she has achieved her goal weight.”

For those who intend on undergoing bariatric surgery, waiting to have breast surgery can have its benefits. Many patients choose to undergo multiple plastic surgery procedures after significant weight loss. In some cases, breast reduction can be combined with other post-weight loss procedures such as liposuction in Portland. Dr. Popowich specializes in post-bariatric procedures and believes that the benefit of combining procedures is minimizing the recovery period after surgery.

Debate about what constitutes a heart-healthy diet has been simplified, thanks to three nutrition experts whose advice is summarized in the November 2011 issue of the Harvard Heart Letter. The focus is on foods you should eat, not nutrients you shouldn’t.

“For most people, getting more of what’s missing will have a larger benefit than limiting certain nutrients,” says Dr. Dariush Mozaffarian, a cardiologist at Harvard-affiliated Brigham and Women’s Hospital and associate professor at the Harvard School of Public Health.

The “shoulds,” according to Dr. Mozaffarian and the two other nutrition experts, include the following:


    4 to 5 daily servings each of fruits and vegetables
    3-plus daily servings of whole, unrefined grain products
    2 to 3 daily servings of low- or nonfat dairy products
    2 to 6 daily servings of vegetable oils
    2 or more weekly servings of fish or shellfish
    4 to 5 weekly servings of nuts and seeds

A handy table in this month’s Heart Letter specifies how much a serving of each food type really is and gives specific grocery-store examples.

On the no-eat list is any food containing partially hydrogenated vegetable oil. The eat-less-of list consists of processed meats, sugary beverages, sweets, and baked goods made with refined grains.

Another useful tip: you can distinguish between healthy and not-so-healthy carbohydrates, most people’s major source of calories, by dividing the grams of carbohydrates per serving by the grams of fiber. An answer less than 10 is good for bread or pastry; aim for less than 5 for cereals.

1.     Never go to a party hungry. It is important not to skip meals or go to a festive function on an empty stomach. If you do, you are more likely to overeat and consume extra calories.

2.    Watch your alcohol consumption. In addition to the dangers of consuming too much alcohol, drinks are loaded with empty calories that can add-up fast. Alcohol can expand your appetite since it causes an increase in the acid level in your stomach, which in turn makes you feel hungrier.

3.    Stay active. People who are more active during the holidays do not gain weight. It is important to squeeze at least 15 extra minutes of exercise into your day. This can easily be achieved by taking the stairs at work instead of the elevator, taking the dog for a walk, or doing something active with your children. The point is, you want to balance your exercise level with your calorie consumption. By doing this, you will not gain that extra holiday weight.

4.    Keep your portions small. When going through the buffet line, do not load-up your plate with giant size portions. Smaller sized samples of your favorite foods will keep you satisfied and will help avoid that overstuffed, uncomfortable feeling.

5.    Make smart choices. To shave calories, go easy on the nuts, cheeses, cream sauces, gravy, butter, and whipped cream. All of these foods are high in fat and will quickly increase your calorie intake. Add more fruits and vegetables to your diet, both offer less calories and are healthier for you. Enjoy just one of your favorite desserts, and stay away from eating handfuls of cookies and other small desserts which you are more likely to over eat on.

Dr. Hertko concluded, “If you keep these helpful tips in mind, when you attend your next holiday gathering you will be armed with the tools needed to manage your calorie intake and avoid those extra holiday pounds. Obesity can lead to potentially serious health problems including heart disease, stroke, and type 2 diabetes. In addition, being obese can affect a person’s overall quality of life which can lead to depression.”

Team captain was Judkins, who assembled the members from a Facebook posting about the contest.

More than 95 people participated in the contest organized by Dynamite to recognize the benefits of its new weight-loss product. The total weight lost by all competitors was more than 1,120 pounds. Dynamite gave $ 200 gift cards to five members, whose names were drawn at random, of the three winning teams.

Dynamite’s 180, introduced earlier this year, has recorded the fastest growing sales of any product in company history.

“People wanted an all-natural weight loss product that works,” said Callie Novak, Dynamite CEO. “Even though there is a lot of competition, most weight loss products don’t work well and most contain harmful ingredients. Pet Obesity Expanding in US

A new nationwide study indicates that pet obesity continues to expand. According to the Association for Pet Obesity Prevention (APOP), almost half of the nation’s dogs and cats are now overweight or obese. The group conducted the National Pet Obesity Awareness Day study in October 2007 and found 43% of all dogs and 53% of all cats were classified as overweight or obese by a veterinary healthcare provider; 10% of all dogs and 19% of all cats were classified as obese.

“The startling fact was the number of obese cats,” comments the study’s lead investigator and founder of APOP, Dr. Ernie Ward. “We knew from a pilot study completed in early 2007 that obese cats were the fastest growing segment. It is alarming that almost one-in-five US cats is now clinically obese. This means we will continue to see an increase in cases of type 2 diabetes, hypertension, and other weight-related diseases.”

The study also looked into pet owners’ assessment of their dog’s or cat’s weight. “We found that the majority of pet owners understand that their pet is too heavy,” notes Dr. Ward. In fact, 53% of dog owners with overweight or obese canines classified their dog correctly and 66% of cat owners with flabby felines stated that their cat was overweight or obese. “As we questioned pet owners further, we discovered that many of them had never been told that their dog or cat needed to shed a few pounds. I believe that in order to win the war on obesity it must begin with bilateral communication: pet owners need to ask if their pet is too heavy and veterinarians need to tell owners when a pet is overweight.”

According to APOP estimates, this study indicates that as many as 32 million dogs and 46 million cats in the US are overweight or obese. Almost 8 million dogs and 17 million cats are thought to be obese.

“Carrying a little extra weight isn’t a stable condition that’s simply annoying or unsightly. Fat is biologically active tissue and an excess amount negatively impacts almost every body system,” states Dr. Ward. “We’re in real danger of raising an entire generation of pets that will live a shorter life expectancy than the dogs and cats we enjoyed as children.”

The nationwide study was conducted in 98 small animal veterinary clinics and included 704 dogs and 283 cats.
